Removes cookie inspection in hook_boot which seems to be causing
certain installs to still cache authenticated page requests.

Thanks to vkareh for pointing this out.

Adds a minimal check in hook_boot to see if local cosign has
stored a remote user, and if so, turns off caching for the page
request. The idea here is to use hook_boot to speculate about the
forthcoming authentication inspection in hook_menu_get_item_alter
and prevent page caching from happening before it gets to that
point.

First stab at D7 support.
Remove webserver auth dependency.

add support to cosign module so that when it is enabled, it requires that ['REMOTE_USER'] is set, and makes that username the admin user, this should prevent issue http://drupal.org/node/117664 from happening
