Remove erroneous return t() (Issue #486122) and fix some outdated text and links on the page as well.

* move filter settings to "per format" settings like every other filter
* store the variables for per format as a multidimensional array
* move the help and delete the menu item associated with settings previously

- Pretty heavy rewrite. There was lots broken in the old version and this fixes some but probably breaks new stuff.

- Removed all the 4.6 compatibility code.
- Fixed some formatting and replaced "s with 's.

4.6 didn't display the admin/settings help as promised, so I was doing it myselg. 4.7 does, however, so I check.
Just to make sure the interface is big and helpful.

The module is now a hybrid, 4.7-based, but workable on 4.6 with a little help from a backport helper

But I've got useful things to add.

- Reworked a lot to enable use as an input validator
and an output filter, as well as the original site-wrapper.
- Added a test and install function to help ensure the HTMLTidy
executable is present.
- Configured so it can optionally use an external tidy.conf file,
and so make use of all the HTMLTidy options. (like clean Word2000)

But I've got useful things to add. Lets see if I have permission to commit

- CSS fix that makes debug mode work better.
- Expanded help text for the 'clean' setting.

- Cleaned up the code a bit more (variable names).
- Added i18n support for _settings() hook.
- Updated 'run twice' functionality to only happen for 'debug mode' users.

meaning the HTML never got modified.  This is all better now.  I also tested
against cvs HEAD, and this module works just fine.

- htmltidy.module now works with Drupal cvs.
- The user has several more choice for configuration options.
- The code is much cleaner and more efficient.
