Needed to revert some changes to use SelectQuery::where() instead of db_and() and db_or(). Resolves issues with profile module.

 - Code refactoring: Cleaning up email send code (much to be done)
 - Enhancements:
 - http://drupal.org/node/111202 (enhance new and updated user email
   notifications)
   - Added %user_email, %google_user, %yahoo_user substitution vars.
   This gives the ability to insert the user's chosen email address, as well
   as provide links to google and yahoo searches for the user's new
   email address (quick way to see if a user is a likely spammer based
   on email address).
   - Updated default email templates to insert %user_email, %google_user,
     and %yahoo_user variables into the sent email, so recipients have
     useful information without having to visit user's account edit page.
 - Settings form:
   - Changed default settings to enable notifications by default
   - Now warns if no roles selected (which would result in no mails being
     sent)
