use standard drupal_get_form instead of custom page callback for redhen

note add form. we were only using the custom callback to ensure we had
a note before calling the note form, so i moved this logic into
redhen_note_form. this fixes an issue were a basic file field could not
upload files via the “upload” button when a note was being added.
