Coder Review fixes, removal of unused code and depracted test frameworks, progress on ldap_authorization tests, and general code cleanup and documentation

Issue #1777420:  automatic deletion of related ldap entries when a drupal user is deleted. (when option is selected and ldap entry is provisioned by drupal)

Mostly simpletest coverage, but fixed some bugs in ldapServer->group* functions and a token function bug.  Almost through with ldap_server module.  Server form validation and warnings and group create delete, and modify functions remain

LdapServer method tweaks and simpletest tweaks.  LdapServer::group* functions are present now, but not tested at all.  Am working through simplests in ldap_servers/tests/ldap_servers_v3.test for test coverage

moved ldap_user and ldap_authentication simpletests to new test model.  Ldap authentication test has 1 fail -- User able to authenticate because of excludeIfNoAuthorizations setting -- which needs to be dealt with after ldap_authorization is in new test framework

trigger, event, synch action reworked to avoid ambiguity, clean up user interface, and separate ldap_user events from generic ldap contexts.  All simpltests working, but not tested beyond simpletests.  Ldap user configuration will need to be updated before using.  Some of this commit had to do with simplest fake server adjustments.

ldap user and ldap server passing simpletests; ldap authorization and authentication need good deal of work.  lots of debugging statements.  need to still bury semaphore function

This commit is largely refactoring of ldap_user to clean up the provision trigger, provision action, and ldap context logic.  It passes all the simpletests, but requires configuring ldap user again.  It also has been tested very little outside of the simpletests and the UI.

Commit is all work on provisioning to ldap.  Its a partial commit, so keep using last commit for now. It passes simpletests, but has some loose ends still.  Refactored provision to ldap event handlers for (1) readability and (2) moving all event handlers into hook_user_insert, hook_user_update, hook_user_delete, and hook_user_login and out of user form submits to avoid repeated synch and provision. Added ldap_user_ldap_provision_semaphore() function to avoid multiple synch or insert in same page load and safely store unhashed user password until hook_user_* needs it.

additional work on provisioning to ldap, event handlers for provisioning and documentation.  cleaned up some inconsistencies in variables and token conventions along the way.  still not particularly useable, but getting close.

getting test coverage working again.  ldap_authentication, ldap_servers working.  ldap user tests almost done.  Added switch for both versions of testing model/classes

committing progress on ldap_user synch interface and simpletests.  LDAP 7.x-2.x still not functional yet.

ldap user with basic function test coverage.  2.0 branch still not likely to be functional, just wanted to commit some of the progress

merge of 7.x-1.0 branch into 7.x-2.0 branch.  Only ldap provision module left in until it can be integrated into ldap_user
