Improved documentation and menu and form title and descriptions. Added more code comments. Added admin settings page validation function. Turned the two admin settings pages into menu tabs.

abuse moderation menu items are not cached but they were being called on all page loads. changed to only load when in abuse moderation path.

147643 fixed issue with multiple forms with same form-id mean form submits don't give the values you wanted.

Fixed case where warn and allow would not work when underlying node or comment had anonymous author.

SQL formatting cleanup.

#167482 pulled from patch from anonymous5190 to reset abuse count when content is edited.
changed abuse node type settings to use human node type names.
wrapped more strings for translation.
small refactor of hook_link() code.
